Monthly Reporting
Agencies are required to submit specific information by the 10th of each month to include; completed Monthly Check sheet, CINS/FINS Report, CMBE Report, IVE Invoice and DJJ Volunteer form. You can find these forms below.

The Checklist is to help you keep track of what needs to be submitted each month.

The CINS/FINS Report, also known as the FNYFS Report is generated from NetMIS and only the signature page needs to be sent.
|
|
The CMBE Report is where you list the money that your Agency has spent on a Certified Minority Business Enterprise in the previous month. Usually, you will have the Business Manager fill out this form.
|
|
Title IVE Invoice is also a report generated by NetMIS. This Invoice lists the youth that are eligible for Title IVE funds. Please make sure you finalize this Invoice! In addition to the invoice you must also send the Notice of Case Action form for each youth listed on the Invoice. If you do not have any youth eligible for IVE that month the report that you will send in will look like this: Blank IVE Report.
